***We require you pet to be up to date on all vaccinations and/or blood titers. Please provide us with certificate of vaccination for distemper/parvo, Bordetella, and Rabies***
Diet
Did you bring your pet's food?
**We require you to individually prepackage your pet's food per meal**
***If you do not provide your pets food there will be a fee of $5 daily to cover the food cost***
****If your pet has diarrhea for more than 48 hours of being at our favility we will give them a prescription diet that is easier on their digestion. There will be a fee of $3 per meal added to your total invoice****
Medication
Is your pet currently on medication?
Will someone other than you be picking up your pet?
If your pet friendly with both men and women?
Does your pet have any allergies to medications, food, insects, etc.?
Is your pet aggressive with other dogs or cats?
Does your pet chew up toys/blamkets and eat them?
Would you like your dog to have a dog bed?
*Owners are financially responsible for broken, damaged, or chewed mattress and/or beds and for any damage done to the kenneling area by their dog.
Are there any sensitive areas on your pet that they do not like touched?
Boarding Rules and Requirements
All dogs should be in good health, free of communicable diseases and written proof from a veterinarian of these current vaccinations is required:
- Distemper/parvo
- Rabies
- Bordetella
Boarding Cancellation policy: 72 hours notice before the reservation is required for cancellations; if less than 72 hours, a cancellation fee of $35 will be incurred
Pet owners are financially responsible for any damage or destruction to the kennel enclosure caused by their pets
Liability Waiver
I agree that if I or any of my associated get injured by an animal at Paws Unlimited Inc., I will not hold Pawsunlimited Inc. liable for any injuries or damages that may occur.
I have disclosed to Paws unlimited all known risks, dangers, and medical conditions associated with my dog(s). I understand that I am solely liable for medical care expenses and damages that result from injuries caused by my dog, or that I will resolve the matter direclyt with the owner(s) of the other dog(s) based on applicable laws. I understand tha tPaws Unlimited may refuse sercies to my dog for any reason it deems necessary, including but not limited to, overly aggressive behavior, overly excessice barking, and illness. I agree that there are inherent risks in sending my dog to any dog boarding kennel, such as illness or injury, and I accept these risks because they are outweighed by the benefits. Paws Unlimited will not be liable for any illness or injury that may occur to my dog(s).
I authorize Paws Unlimited to take actions they deem as necessary to ensure the health, well-being, and safety of my dog(s), and to take reasonable action to resolve any medical problems that may arise while my dog(s) is in their care. I agree to assume full financial responsibility for any and all expenses incurred as a result of their actions. I understand that Paws Unlimited staff is expected to make a resonable attempt to contact me before incurring such expenses, that they may need to incur such expenses if they can't reach me, and that an emergency situation could arise where it would not be feasible for them to contact me before the expense is incurren. I understand that, as is the norm in the pet care industry, under no circumstances will Paws Unlimited be liable for damages beyond the replacement value of my dog(s). I waive all claims or actions against Paws Unlimited, relating to the care, control, health, and/or safey of my dog(s) arising while my dog(s) is in their care.
I certify that:
- I am the legal owner of the dog(s) I am sending to Paws Unlimited
- That my dog(s) is currently and properly licensed.
- That, to my knowledge, my dog(s) has not been exposed to kennel cough, distemper, parvo, or rabies within the past thirty days.
- My dog(s) has been vaccinated as indicated in the "Rules & requirements" section
I understand that an electronic signature is legal and binding and that by submitting this form I am submitted my electronic signature.
